State employees and retirees/surviving beneficiaries with State Health Benefit Plan (SHBP) coverage will receive a Form 1095-C from the State of Georgia in early March 2017. This form fulfills the IRS filing requirements of the Affordable Care Act and provides certain SHBP participants with employer-provided health care coverage information for your individual tax return filing.
